Have recently built this amplifier. It was difficult to find a suitable non-SMD  IC...
The PCB is double sided, somewhat crudely manually routed, but it works well.
The input signal is distributed to the amplifiers. The 220 Ohm resistors are effectively in parallel, presenting a 55 Ohm impedance to the input signal.
This is good enough. The amplifiers have a voltage gain of 2. The 51 Ohm resistors represent the source impedance to the output. 47 Ohm will work just as well, if not better. If terminated with 50 Ohm, the output voltage is divided by half. Therefore, input voltage equals output voltage, but decoupled and buffered.
